The practice is currently accepting new patients.
If you wish to register with the practice, please firstly check the map on the ‘Practice Boundary’ page under the ‘New Patients’ tab at the top of this page, to ensure that you live within our catchment area. If you live within our catchment area, please make an appointment for a ‘New Patient Medical’. This is a 20 minute appointment with one of our Healthcare Assistants. The purpose of this appointment is to allow us to understand your past medical history to allow all our healthcare professionals to provide you with the best possible care, as it can take a long time for your medical records to arrive from your previous surgery. This is not a process administered by ourselves but by NHS England, we have no control over how long it takes for your medical records to arrive. If you live outside of the catchment area, please consult the Out of Area Patients page for more information as to wther we can accept you as a patient. call the surgery to see whether the GP surgery Partners are willing to accept you as a patient. They may be willing to accept you but with the understanding that Home Visits will not be made for any reason (this will be explained to you at the time of your New Patient Medical). Patient registrations are entirely at the discretion of the Partners – Bedworth Health Centre reserves the right to reject new patient registrations but cannot refuse to accept you without reasonable grounds for doing so. More information for patients who live outside of our catchment area, but wish to register as a patient, can be accessed through the 'Out-of-Area Patients' page.
All patients registering for the surgery must complete a ‘GMS1’ form and, ideally, provide both photo ID and a proof of address*. The GMS1 is a standard NHS form known as the ‘Family doctor services registration’ form. You can complete this in advance of your appointment here^. Please only complete this form once you are sure that you are eligible to register for the practice (i.e. you live within the catchment area or have confirmed that the practice will accept you, if you live outside the catchment area). On arrival for your appointment, please let the Receptionist know if you have completed this form online, to save you having to complete it again. The Receptionist will print the form and either ask you to hand it to the Healthcare Assistant or will give it to the Healthcare Assistant on your behalf. You will be required to sign this form to confirm the details you have provided are correct. We request you arrive at least 10 minutes prior to your appointment. This is to allow you enough time to complete practice specific paperwork prior to your appointment – we are endeavouring to publish this form on our website to allow you to complete it in advance of your appointment.
^Please note that by sending this form you will be transmitting information about yourself across the Internet. Although every effort is made to keep this information secure, no guarantee can be offered in this respect.
*We can accept the following ORIGINAL documents as a proof of address:
- Bank Statement / Credit Card Statement / Utility Bill (Gas / Electric / Water / Landline Telephone**) – these must be dated within the last three months for them to be acceptable.
- Mortgage Statement / Council Tax Statement / Official correspondence from HMRC – these must be dated within the last 12 months for them to be acceptable.
- Photocard driving license (please be aware that if you are registering for GP Online Services, we can only accept a photocard driving license as a photographic ID OR proof of address, NOT both - you would need to provide either an additional photographic ID or an additional proof of address for us to complete your GP Online Services registration)
**Please note we are unable to accept Mobile Phone bills as a proof of address. Similarly we are unable to accept broadband / television subscription (e.g. Sky / BT / Virgin Media) bills.
