It is really important that we have up to date contact details for yourself so that we are able to get in touch with you if we need to (this is especially relevant for emergency situations (e.g. abnormal test results etc.)). If you change your surname, address or telephone number please either request a ‘Change of Personal Details’ form from Reception or complete the below form.

If you move to a new address which falls outside of the Practice’s catchment area, you will need to discuss with the Practice whether or not we are able to retain you as a patient, or whether you will be required to register with another surgery which is closer to your new address.
