It is important we have up to date contact information for you. This is very relevant for urgent circumstances (e.g. abnormal test results). If you change your surname or address, complete the below form.
If you move to a new address which falls outside of the surgery’s boundary, you will need to register with a surgery closer to your new address – we do NOT register prospective patients who live outside of the boundary, nor do we retain current patients who have moved outside of the boundary (in this case, you will have 28 days from the date of informing us of your new address, before you are de-registered as a patient – the surgery has no obligation to see you after this date).
